DD Form 1898: This multi-part form bearing control numbers is solely controlled by Defense Logistics Agency (DLA) Document Services who is responsible for the printing, stocking and distributing the form. DLA (Energy) pays all printing costs, therefore free to the using Services and commercial Into-Plane contractors (locations where there is not a DOD military base for US military to land and refuel). AMC will no longer order, stock, and distribute this form. Please contact DLA Document Services to order this form.

The Printing Management Division (PMD) accepts and processes FS Form 7600s (United States Government General Terms & Conditions (GT&C) through GPO. FS Form 7600s are to be sent directly to PMD, through the following email address. FS Form 7600s cannot be sent directly from the customer to GPO. The FS Form 7600 will be received by PMD and then PMD will send the FS Form 7600 to GPO. GPO will then provide PMD with the signed and accepted FS Form 7600A and/or FS Form 7600B. PMD will in turn provide the signed FS Form 7600A and/or FS Form 7600B, to the customer.

PMD will be processing DD Form 448 (Military Interdepartmental Purchase Request (MIPR)) through GPO. MIPRs for print requests are to be sent directly to PMD, through the following email address. MIPRs cannot be sent directly from the customer to GPO. Remove The MIPR(s) will be received by PMD and then PMD will send the MIPR to GPO. GPO will then provide PMD with the signed DD Form 448-2. PMD will in turn provide the signed DD Form 448-2 to the customer. When submitting the MIPR to PMD, please add the below address in the TO block (Block 7).

PMD will in turn provide the signed FS Form 7600A and/or FS Form 7600B, to the customer.

DD Form 2542 (Certificate of Appreciation for Service in the Armed Forces of the United States) is a controlled form. The Army is the office of primary responsibility for DD Form 2542.

The Department of Air Force obtains DD Form 2542 through the Air Force e-Publishing Warehouse Management System (WMS).

The Headquarters Department of the Navy and the Headquarters United States Coast Guard order directly through PMD. The Headquarters for both the Navy and the Coast Guard will provide distribution.

Army customers contact your organization's Forms Management Officer (FMO) to obtain the DD Form 2542. Your organization FMO can establish an Ordering Portal account through MDD. If your organization doesn't have an Ordering Portal accounting, please contact MDD directly to open an Ordering Portal account either via: email address or via phone: 314-592-0910/DSN 693-9606.

Another option to obtain the DD Form 2542 is to contact PMD directly, through the following email address. When reaching out to PMD, please provide a DD Form 282 with the below fields completed.
